Cougar Volleyball Club Training Camps Medical Release Consent

By checking the boxes below, I acknowledge and agree to the terms outlined in the medical release consent form for my child's participation in the Cougar Volleyball Club Training Camps:

I give my consent for above named athlete to participate in the Cougar Volleyball Club Training Camps.

I am responsible for informing the coaching staff of any medical conditions or allergies that may affect my athlete's participation.

In the event of an emergency, I authorize the camp staff to seek medical treatment for my child if necessary.

I understand that I will be contacted as soon as possible in the event of an emergency.

I release the Cougar Volleyball Club, its coaches, and staff from any liability associated with injuries or accidents that may occur during camp participation.
