Cougar Volleyball Club Training Camps Medical Release Consent

By checking the boxes below, I acknowledge and agree to the terms outlined in the medical release consent form for my child's participation in the Cougar Volleyball Club Training Camps:

I give my consent for above named athlete to participate in the Cougar Volleyball Club Training Camps.

I give my consent for the Cougar Volleyball Club, to take video and pictures of my athlete for use on their (and their training partners) social media and its website that may occur during Cougar Volleyball Club Activities.

I am responsible for informing the coaching staff of any medical conditions or allergies that may affect my athlete's participation.

In the event of an emergency, I authorize the camp staff to seek medical treatment for my child if necessary.

I understand that I will be contacted as soon as possible in the event of an emergency.

I release the Cougar Volleyball Club, its coaches, and staff from any liability associated with injuries or accidents that may occur during camp participation.
